Be advised Form 1095-C will not be mailed and is available upon request from Claiborne Parish School Board.  Form 1095-C—under the Affordable Care Act (ACA) is used to report health coverage offers for full-time employees and retirees with coverage.  To request a copy of the form, please contact Micki Turner at (318) 927-3502. Forms will be furnished within 30 days of request.
